Due to our continued growth, we are looking for caring, reliable and committed Support Workers to join our team. The Role As a Support Worker, you will: * Provide person-centred care and support in line with individual care plans. * Support individuals with personal care, medication and daily living. * Encourage independence, choice and decision-making. * Support individuals to access the community and develop social relationships. * Promote healthy lifestyles and wellbeing. * Work closely with Team Leaders, Care Managers and other professionals. * Contribute to person-centred plans, reviews and support goals. * Act as a Key Worker where required. Most importantly, you must be caring, dependable, patient and committed to making a positive difference to the people we support. A full UK driving licence is desirable but not essential.
